* NURSING PROCESS (ADPIE)
The nursing process is defined as a systematic, rational method of planning that guides all nursing actions in delivery holistic and patient focused care. The nursing process is a form of scientific reasoning and requires the nurses critical thinking to provide the best care possible to the client. These are Assessment, diagnosis, planning, implementation and evaluation.
- Assessment:
It involves critical thinking and data collection
Subjective (symptom/covert) data
Objective (sign/overt) data
- Diagnosis
It involves/needs clinical judgment based in the assessment to assist in the planning and implementation.
Purpose: to identify the problem of the patient.
Prioritization: Maslow hierarchy of need ABC
- Planning
The goal and outcome are being formulated; meaning it’s should be SMART
S- specific
M- measurable
A- attainable
R- realistic
T- time Oriented
- Implementation
The implementation of nursing intervention outlined into plan of care.
- Evaluation
To evaluate if the desired outcome has been met, partially met or not met
